I confirm I do not have an active infection, open wound, or cold sore in the brow area.

I confirm I have not taken oral isotretinoin (Accutane) within the last 6–12 months.

I confirm I have not had aggressive resurfacing or laser on the brow area within the last month.

I agree to avoid heavy sweating, swimming, intense sun exposure, and facial treatments for 24–48 hours after service.

I understand the risks associated with brow henna and waxing, including temporary redness, irritation, uneven fading, and possible allergic reaction. I confirm that the information I provided is accurate and complete to the best of my knowledge. I consent to the requested brow services.
